Requesting Deletion of Your Data
We take your privacy seriously and support your right to request deletion of your personal information where appropriate. Because we operate under the Health Insurance Portability and Accountability Act (HIPAA), there are specific legal requirements that limit deletion of certain health-related data.
🔹 How to Submit a Data Deletion Request
To request deletion of your personal data, please send your request to: whill@frontierneuro.com
Subject: Data Deletion Request
Please include:
- Your full name
- The email address associated with your account
- Any relevant identifiers (e.g., DOB)
🔹 What Can Be Deleted
Upon request, we can delete:
- Your portal account
- Non-clinical personal information (e.g., preferences, usage logs)
🔹 What Cannot Be Deleted
Under HIPAA, we may be required to retain certain health-related information in order to:
- Maintain an accurate medical record
- Comply with federal or state laws
- Support care delivery, audits, or legal obligations
- If your data is part of a provider’s designated record set or protected under a provider’s policies, your request may be forwarded to the responsible healthcare provider for review.
🔹 What to Expect
We will acknowledge your request within 10 business days. If applicable, we may ask you to verify your identity. Eligible data will be deleted within 30 business days, and you will receive confirmation. If any data must be retained for legal or compliance reasons, we will notify you.
